What Is a 100% VA Disability Rating?
A 100% disability rating is the highest possible number a veteran can receive from the VA. The VA assigns ratings based on the severity of your disability, with higher percentages indicating greater impacts on your quality of life and ability to function. The higher the rating, the more you’re eligible to receive each month in disability compensation.
You can receive a 100% rating for a single disability or a combination of multiple disabilities, if their combined impact is considered severe enough for you to be totally disabled.
How to Get 100% Disability Rating
While it can be tricky to obtain a 100% VA disability rating, there are multiple pathways to securing the same or similar benefits:
- Direct rating: You can be assigned a 100% rating for a single disability. Although most individuals with a direct 100% disability rating cannot work, there is no requirement preventing you from working.
- Combined ratings: You can achieve a 100% rating by receiving a combined disability rating of 95% or higher for two or more conditions, which the VA will round up. Like with individual conditions, there is no work requirement for receiving 100% disability through a combined rating. - TDIU: Total Disability based on Individual Unemployability, or TDIU, pays benefits at a 100% disability rate to veterans who are rated at a lower percentage but still are unable to work. Vets can be eligible for TDIU if they have either one service-connected disability with a 60% or higher rating, or multiple conditions with a combined 70% rating and one rated at least 40%.
- Special monthly compensation: Veterans with specific, severe disabilities are often entitled to special monthly compensation, which provides additional payments to cover care and assistance. Some types of SMC may be available to those with a disability rating below 100%.
What Are Some Common 100% Conditions?
Whether it’s physical, mental, or otherwise, any sufficiently debilitating condition can lead to a 100% VA disability rating. However, some disabilities are more likely than others to qualify, including:
- Blindness
- Limb loss and amputations
- Chronic pain
- Paralysis
- Crippling mental health conditions, such as severe PTSD and major depressive disorder
- Certain types of cancer, particularly those resistant to treatment
- Heart failure
- Respiratory issues
- ALS
- Spinal cord injuries and degenerative conditions
What Benefits Can Be Obtained for Those With a 100% Disability Rating
In 2026, veterans with a 100% disability rating and no dependents can receive monthly payments of $3,938.58. Vets with dependent spouses, children, or parents can receive additional compensation. Certain veterans are also eligible for special monthly compensation payments.
Those on VA disability benefits are eligible for free or low-cost health care through the VA. Vets with 100% disability ratings may also qualify for free dental, vision, and hearing care.
For those who wish to continue working, Veteran Readiness and Employment, or VR&E, services can help with job training, placement, and other resources.
VA disability benefits can also extend to your dependents. Spouses, children, and caregivers may qualify for VA health care benefits. If you pass away, your surviving family can receive burial costs, educational benefits, and additional financial support through VA Dependency and Indemnity Compensation, or DIC, or a Survivors Pension. Your spouse may also be eligible for housing assistance.
What Should You Do if You Don’t Qualify for a 100% Rating?
If you did not receive a 100% disability rating from the VA, you can appeal the decision and seek to reverse it. If you have additional medical evidence or a second opinion from a health care provider that could strengthen your case, you can file a supplemental claim to include the new supporting information.
The second method of appeal is to request a higher-level review, which can have a more experienced adjudicator look over the initial decision. If that step is unsuccessful, you can file a formal appeal with the Board of Veterans Appeals, triggering a hearing with a Veterans Law Judge.
